Reports to: Case managing nurse, client and/or responsible party, office and management personnel.

Purpose for position:  A Registered Nurse will perform direct nursing services to clients of Accurate Home Care, to help each client maintain his/her highest level of performance and independence. 

Principal Responsibilities:

Direct Client Cares (include but are not limited to):

·         Responsible for implementing the nursing process including assessment, diagnosis, planning, goal setting, implementation and evaluation, in delivery of professional and paraprofessional services to private duty and/or pediatric home care clients

·         Understand and comply with the client’s Home Health Plan of Care and Physician’s Orders

·         Delegate appropriate nursing functions according to State Licensure and Accurate Home Care’s policies and procedures

·         Communicate with physician and other health care providers regarding the health status of client and follow through with physician or prescribed instructions – Communicate appropriately with RN Case Manager

·         Participate in care conferences

·         Lift, turn, position and transfer up to 50 pounds of weight and use mechanical lift when available

·         Demonstrate safe and appropriate techniques while providing cares 

·         Maintain compliance with county, state and federal regulations affecting home health care.

·         Cleanse and disinfect reusable medical equipment and supplies

·         Monitor, observe and report changes in client’s health or behavior

·         Provide supervision and observation for client safety

·         Assist in orientation, training and assessment of new staff as needed

·         Accompany client to medical appointments

·         Understand that regular and consistent attendance and timeliness are imperative  – if attendance or timeliness becomes a problem, it is grounds for termination

·         Occasionally perform other functions deemed necessary by the supervisory team

·         Adhere to the policies in effect at Accurate Home Care

Documentation and Reporting

·         Accurately document cares provided in home chart – use only black ink and print legibly

·         Document daily activities in Care Notes

·         Accurately document days and times for cares provided on timecard

·         Report to Office Manager any accidents or injuries involving the client within 24 hours of the occurrence

·         Immediately report suspected incidents of client abuse or neglect to the office manager

·         Report any unsafe client behavior to the case managing nurse and office manager

Communications:

  • Verbal communications with the client, client’s responsible party, and Accurate Home Care staff must be conducted in a respectful and professional manner
  • Communicate scheduling needs and conflicts with appropriate party (Staffing Coordinator or Client/Responsible party)
  • Notify office if unable to work scheduled hours as soon as you become aware of your unavailability – A minimum 8 hour advance notice is requested in all cases
  • Submit vacation requests at least two weeks in advance
  • In the event of resignation from the company, a two week notice is requested

Physical Requirements:

  • Must be able to lift, turn, position and transfer up to 50 lbs
  • Must present a negative TB skin test or chest x-ray

Position Specifications:

  • Must possess a valid driver’s license and proof of insurance if driving on company time
  • Must have the ability to get to the client’s residence on time
  • Position will not exceed 40 hours  – in the event that more hours are needed, approval from the office is required prior to working overtime
  • FLSA definition: Non-Exempt.  (This position is paid hourly)
  • Working conditions are normal.  Does include some exposure to infectious wastes, toxic chemicals, needles and bodily fluids.

Educational/Experience Qualifications:

  • Current RN License for the state of Minnesota
  • Successful completion of competency test
  • Participation in inservices and continuing educational programs as required by federal and state regulations
  • Some cases may require additional training on specific medical equipment, Accurate will reimburse for such classes after 30 days of employment
  • Individual must be able to show compassion and empathy of persons who are chronically ill, or otherwise disabled, and demonstrate an understanding and maturity with respect to the responsibilities and expectations of the position.  Individuals must demonstrate the ability to read, write and carry out written as well as verbal directions in English.
