Care Managers

A Care Manager that is either a Registered Nurse (RN) or a Licensed Social Worker (SW) will be assigned to facilitate the transition to our service.  The Care Manager will oversee your case; ensuring that field staff is trained in the client care needs, to develop the care plan and to collaborate with the physician and other health care providers.  The Care Manager is the person to whom you should provide any feedback to regarding the field staff that work in your home.  They are there to ensure your satisfaction with our quality of care.
